Coaching Philosophy *- Coaches are asked to sign a copy of this form and give to their respective school office to keep on file. In essence it is a "Good Sportsmanship" agreement between a coach and the league.
Game/Match Reschedule Procedure *- Here is the format and timetable that must be adhered to when a school/parish requests a game/match to be rescheduled due to a school or parish related conflict.
MAISL Incident Report *- We would ask that a school Administrator, MAISL Rep, Game Official or Coach submit this document if there is a serious incident or a player injury at a MAISL sponsored activity. This form should not be submitted by a parent.
Petition to Combine Form *- You must use this form if you are combining students from more than one school to form a team. This can be done after you have exhausted all possible options within your school/parish.
Player Substitution/Addition Form *- Use this form if you must substitute a player from a lower grade level team to a higher grade level team. You can also use this form if you are adding a player to a team after the Principal Verifed Roster has been submitted
Principal Verified Roster *- This is the "official" form used when a school/parish submits a team for play in a MAISL sport league. (Revised 11-19-08)
Sport Submittal Form *- Schools are to use this form to submit - Gym/Field Use, No Play Dates and Team Registration
