MAISL Athletic Forms

Standard forms used by MAISL (Madison Area Independent Sports League) include but are not limited to:
 

Principal Verified Roster - This form must be submitted prior to each sport season

Petition to Combine - Submit this form if a team consists of students from multiple schools 

MAISL Incident Report - To be submitted by a referee, coach, principal or MAISL Rep

Player Substitution/Addition Form - Use this to augment a roster with lower level player or if there is a new player

MAISL Game/Match Reschedule Procedure - If a game/match reschedule request is made you must follow this procedure
